Based on the selections you made on the Death Scenario Assistance Chart, the following document provides you with information on completing your Death Scenario transaction. This information can also be found in the State of Florida's procedure, TL-18 (flhsmv.gov/pdf/proc/tl/tl-18.pdf).
1. Required Documents
- Florida Certificate of Title* showing the deceased lienholder
*If the title is electronic, it must be printed. If the title is lost or destroyed, HSMV 82101 (Application for Duplicate Title) completed by the recipient(s) (as named in the deceased lienholder's will) as lienholder.
- HSMV 82139 (Notice of Reassignment of Lien) - Sections 1 and 3 must be completed by the recipient(s) indicated in the lienholder's will, reassigning the lien to the new lienholder
- A photocopy of the lienholder's death certificate
- A photocopy of the lienholder's will

Valid, unexpired U.S. driver license/ID card - We will also accept valid U.S. or foreign passports*, or Canadian driver license/ID cards.
*If presenting a foreign passport, the State of Florida requires one of the following unexpired documents to be presented as proof of legal presence:
- A passport issued by the government of another country with a stamp or mark from the United States Department of Homeland Security
- A Permanent Resident Card
- An Immigrant Visa
- An I-94
2. Processing Information
- If the title is electronic, the cost to print it to paper is $10.00.
- If the title has been lost or destroyed, the cost for a duplicate title is $85.75.
- The cost of reassigning the lien is $74.25.
Our office accepts cash, check, debit cards, and credit cards for in-person transactions. A $2.75 processing fee applies to debit card payments, and a 2.75% processing fee (minimum of $2.75) applies to credit card payments. Our office does not retain any portion of these fees.
